Payment is due at the time of service. An estimated co-pay will be explained once eligibility is checked. Final patient responsibility may vary depending on how your insurance processes the claim. Any remaining balance will be due at that time.

NOTE: In accordance with our Financial Responsibility Agreement, payment is due at the time of service. For appointments scheduled after hours, via Telehealth, or at our Springville location, patients with a required co‑pay or deductible are asked to contact our main office during regular business hours to place a payment card on file prior to their appointment.
